How do I get started?
Call or come into the office to schedule an intake appointment. The intake appointment takes approximately 30 minutes.
During this appointment, we will enroll you for services and determine what, if any, fees will be charged.
After the intake, you will have an assessment to review problems and strengths and develop a short-term treatment plan. This appointment lasts about 2 hours.

What are the next steps?
After the assessment, a counselor will be assigned. This person will be your primary contact with CBHS.
During the first visit with a counselor, a service plan will be developed based on your needs. This service plan will establish your recovery goals.
Once your goals have been established, you will begin your journey of recovery with CBHS at your side.
What does it cost?
Some services are covered by AHCCCS, others by health insurance. Our fees are based upon family income and size. Fees are determined by a sliding fee schedule.
What forms of payment are accepted?
Cash, check, money orders, credit cards.
Is my information kept confidential?
Our policy is to carefully guard any personal information you share with us, and confidentiality is strictly enforced at CBHS. Information about clients is kept confidential in accordance with legal and professional ethical requirements. No information can be related to any outside person or agency without written permission by the client with few exceptions.
However, there are exceptions to a client's legal privilege to confidentiality:
(1) when a client threatens or is at risk of harm to him/herself (e.g. suicidal), (2) when a client's statements or actions threaten to harm others, (3) when there is reasonable suspicion of child abuse or neglect, (4) when there is reasonable suspicion of abuse or neglect of a dependent adult, and (5) when records are court-ordered by a judge compelling disclosure.
My services are court-ordered. What do I need to know?
If you are here for court-ordered services, you MUST keep your appointments. If you do not show up for your appointments we are required by law to notify the court, probation or parole office.
If you were referred by the court your court obligation will not be satisfied until you have completed your counseling and paid for services. Your certificate(s) of completion will be withheld until your account is paid in full.
How do I file a complaint?
A written or oral grievance may be filed by a client, guardian, human rights advocate, human rights committee, State Protection and Advocacy System, designated representative or any other concerned person, within 10 days of such activities being of concern that constitute the definition of a grievance. The grievance form can be obtained at the front desk of any CBHS location.
How do I obtain a copy of my file?
Fill out a file copy request form, which can be obtained at the front desk of any CBHS location. You will be provided with a copy within (30) days from receipt of the request.
What is the prescription refill process?
Here are a few suggestions for making your refill process as simple and quick as possible:
REFILLS SHOULD BE CALLED IN 5 TO 7 DAYS BEFORE you run out of your current quantity. This is to ensure that the new prescription is ready for you prior to running out.
CALL YOUR PHARMACY FOR REFILLS -- both Safeway and Walmart have 24-hour request lines:

This is a very simple procedure and only takes a few minutes. Even if you have to call after business hours, you can still request a refill.
Walmart has also set up a refill request section on their website (walmart.com). You will have to register, but the process is simple and once you've signed up, the refill request section is easy to navigate.
REMEMBER THAT EVEN IF YOUR BOTTLE SHOWS THAT YOU HAVE ZERO REFILLS, YOU CAN STILL CALL THE PHARMACY FOR REFILLS. Once you've called in your refill, the pharmacy will fax a request to CBHS. The doctor will fax it back to the pharmacy with your refills.
PLEASE CALL THE PHARMACY, NOT CBHS to verify that your prescription is ready, approximately 48-hours after placing your refill request with the pharmacy. The only contact that will be made by CBHS is if there are questions or concerns about your refill request.
FOR CONTROLLED PRESCRIPTIONS (like Concerta, Adderall, etc.) plan for the written prescription to be ready about 4 to 5 working days after your request is called in to CBHS.
When you call our office, ask for the med request line and leave your information on the voice mail. The voice mail is checked every day at 3pm, so the prescription is usually mailed to CBHS the following day. You will be contacted when it arrives and is ready to be picked up at the front desk.
